Payment at the time of service is required. For your convenience, we accept payment by cash, check, MasterCard, Visa, and Discover.
(There is a processing fee for returned checks.) The receipt we give
you at the end of each visit contains all the information needed from
the provider for your insurance company to process your claim. If you
have any questions or concerns about your insurance, please let us know
and we will be happy to assist you. If hospitalization is required, we
will submit insurance claims for your hospital care.
We do participate with many health insurance companies. (See the Insurance listing.)
As participating providers we will submmit your insurance claims for
you however, the basic responsibility for payment is yours.
All Co-Payments, Deductibles, and Non-Covered services are due
at the time of your office visit. For inquiries about your covered
benefits, please call the Member Service phone number on your insurance
card.
