Here are a few things you need to know before your first appointment:
- Confirm you have submitted your new patient enrollment forms, including your immunization records
- Visit Our Office page to find directions to our practice and parking instructions
For every appointment:
- Bring your list of medications so your care team can review them at your appointment
- Bring a picture ID and health insurance information (See a list of our covered insurance carriers)
Think about what you need to tell your care team (your medical history, past treatment, hospital visits, surgeries) and any questions you may have for them. Make a list and bring it to your visit.
